Terms and conditions for submitting an application - GP registrar single employer model campaign:
- These terms and conditions govern the submission of an application to the Queensland Health GP REGISTRAR (SEM) Campaign including your access to and use of the Form and related web pages. By using and/or submitting the Queensland Health GP REGISTRAR (SEM) Application Form (the “Form”), you agree to be bound by these terms and conditions.
- For the purposes of these terms and conditions, a reference to:
- ‘personal information’, is a reference to personal information as defined under the Information Privacy Act 2009 (Qld);
- ‘Queensland Health’, is a reference to the State of Queensland acting through Queensland Health (the department);
- ‘Hospital and Health Service’ or a ‘HHS’ is a reference to a Hospital and Health Service established under s 17 of the Hospital and Health Boards Act 2011 (Qld); and
- ‘a relevant HHS’ is a reference to a HHS that is involved in the GP REGISTRAR (SEM) Campaign.
- In submitting the Form:
- you must complete all sections of the Form as indicated. If you do not complete the Form in its entirety, your suitability for a GP REGISTRAR (SEM) position may be unable to be assessed.
- you warrant that the information and supporting documentation you submit on this Form is true and correct at the time of submission.
- you warrant that you have not submitted the Form on behalf of any other person.
- you must only submit one Form per application round. If you submit multiple Forms in this online system within the period outlined in clause 4.a, your additional applications will be withdrawn and will not be considered for a position as part of the current 2026 campaign. Note: The Form allows you to list multiple site preferences for consideration.
- The information submitted on the Form will be retained by Queensland Health and/or a relevant HHS for:
- a period of up to 12 months; or
- if you become an employee of Queensland Health, indefinitely and in accordance with legislative requirements
- After the period outlined in clause 4.a above has expired, you will need to submit a new Form to be considered for future SEM opportunities with Queensland Health.
- You must advise the Office of Rural and Remote Health as soon as possible if, within the period outlined in clause 4.a. above, there are any changes to your medical registration details nominated in the Form.
- In accepting your Form, Queensland Health makes no representations that:
- at the time the Form is submitted, or any time in the future, that there is a suitable position or any position, available to you, or that you will be considered for a position;
- by submitting your Form you will be notified of any or all appropriate vacancies, offered an interview in relation to a vacant position, or be offered a position. If you are recruited through this online system, Queensland Health will not pay any fees for recruitment activities, to an external agency or third party.
Collection, use and disclosure of your personal information
- Personal information you provide in the Form will be used by Queensland Health, a relevant HHS or their agents for the following purpose:
- to determine your suitability for employment with Queensland Health and in a relevant HHS; and
- for associated employment related purposes.
- Queensland Health and/or a relevant HHS may use your information for workforce planning, statistical or research related purposes, however, personal information will be de-identified for these purposes.
- The nature of the GP REGISTRAR (SEM) CAMPAIGN means that personal information provided by you on the Form will be used by Queensland Health, a relevant HHS, or their agents for the following purposes, and by submitting the Form you consent to the following:
- The Form is hosted on a third-party platform who will have access to your personal information for the purpose of administering the platform;
- Queensland Health and/or HHSs will access and use the personal information you provide to:
- determine your suitability for employment, including disclosing your personal information to and collecting additional personal information from professional and regulatory bodies to verify your qualifications and standing;
- to assess your application, including disclosing your personal information to members of recruitment selection panels which is likely to be comprised of persons and organisations external to Queensland Health and the relevant HHS such as a primary care provider participating in the GP REGISTRAR (SEM) PROGRAM;
- to confirm your placements with specialist medical colleges for workforce planning purposes (this is likely to include providing personal information such as your name, date of birth and workplace location and/or employer) to the specialist medical colleges to validate your eligibility for the role you are applying for;
- collect further personal information about you from professional and regulatory bodies and specialist medical colleges to validate your eligibility and/or suitability for employment. Note: your selection to a SEM position by the Recruitment Panel is merely a recommendation for employment and your application remains subject to pre-employment checks and processes.
- to confirm placements or selections with other Queensland Health facilities;
- other purposes related to administering your involvement in the GP REGISTRAR (SEM) PROGRAM;
- If you are appointed to a GP REGISTRAR SEM role:
- Queensland Health and a relevant HHSs may exchange your personal information for employment related purposes and as required to effectively administer the GP REGISTRAR (SEM) PROGRAM;
- Queensland Health will disclose your personal information to the Commonwealth Department of Health, Disability and Ageing in relation to your involvement in the GP REGISTRAR (SEM) PROGRAM. This will include information collected from the Form as well as other information collected about you from your employment in a GP Registrar SEM Role;
- Queensland Health and/or a relevant HHS or their agents may use your personal information to evaluate the GP REGISTRAR SEM program.
- The collection, use and disclosure of your personal information as outlined above is required for you to be able to express interest in, apply for or be appointed to a GP Registrar role.
Storage and security of your personal information
- For information about how Queensland Health secures and protects your personal information, how to access or correct your own personal information, or how to make a complaint about a breach of the Queensland Privacy Principles and learn how such a complaint will be dealt with, please refer to:
- Queensland Health’s Privacy Policy; or
- the Privacy Policy of the relevant Hospital and Health Service, you can access HHS Privacy Policies by following the links on the About Hospital and Health Services webpage.
- While Queensland Health endeavours to ensure that the online transmission of the Form containing your personal information over the internet is secure, the inherent nature of the internet means that there is a risk that your information may be viewed or intercepted by third parties. Accordingly, submission of your application via the online Form is at your own risk and Queensland Health accepts no responsibility or liability for any unauthorised access to your information contained in the Form when it is submitted online over the internet.
- When you submit the Form online, you can expect to receive an acknowledgement from Queensland Health that the Form has been sent, on the screen and via a confirmation email, following submission. Queensland Health accepts no responsibility or liability if this acknowledgement does not appear or we do not receive your online Form.
- Queensland Health holds your application data at a secure offsite facility within Australia.
