What documents do I need?

All document uploads are mandatory and form part of the assessment of your application. For a checklist of the documents required to be uploaded to your application, download the document checklist [PDF 308.84 KB].

Can be completed electronically but must have a handwritten (wet) signature:

  • Criminal History consent form.

Can be completed and signed electronically:

  • TB risk assessment process
  • Consideration of serious disciplinary action form
  • CV – if using the Queensland Health template
  • Vaccine preventable diseases (VPD) form

If you have previously completed an RMO application, use the same login details (email address and password) for documents to pre-load to your new application. If these documents are not current or need to be updated, upload the new form over the top of the submitted document and click save at the end of the page - this will override the previously submitted document.

All appointments are subject to the employing facility undertaking relevant pre-employment checks. This will include verifying the information you have supplied in your RMO application. Your employer will advise of any additional information / documentation requirements to complete relevant pre-employment checks.

Medical qualifications

A copy of your medical degree, if your degree requires English translation, you can find out more on the AHPRA website. Certification will be require before uploading your degree. Note that if your medical degree needs translation, this must be done prior to the certification.

Medical Board registration

If you are have registration or in the process of obtaining registration with the Medical Board of Australia (MBA). There are different types of registration categories available depending your level of training, level of experience and where medical qualifications were obtained. Your MBA registration does not require certification.

Curriculum Vitae (CV)

Our preferred template for CVs is the Queensland Health Medical practitioner CV template [DOC 319.5 KB]. It outlines all the items you need to include to submit an application with us.

If you’re using your own version, make sure it includes a 'verification statement’ and is signed and dated. For example, I verify that the information contained within this Curriculum Vitae is true and correct as at <insert date> and signed. The signature can be either electronic or a handwritten (wet) signature.

Cover Letter

There is no requirement to submit a cover letter with your application. If you wish to do so, make it part of your CV .

Proof of identity

Current employees are only required to provide one type of photographic ID with a signature and date of birth, preferably a current passport or Australian drivers licence.

New applicants must provide 3 current identification documents from the list below. One must be a type of photographic ID with a signature and date of birth, preferably a current passport or Australian drivers licence.

Identity documents you can use include:

  • current passport
  • Australian drivers licence
  • Australian student photo ID
  • Australian proof of age card
  • Australian citizenship certificate
  • birth certificate
  • health care card
  • Medicare card
  • pension concession card
  • utility bill, for example water, gas, electricity
  • working with children check (Blue Card)
  • NDIS card.

IMG applicants, local documents issued by equivalent overseas authorities may be accepted. When the name shown on the documents differs from the person’s current name, the person is to provide sufficient evidence of their name change (e.g. certified copy of marriage certificate). Note that if your documents needs translation, this must be done prior to the certification.

Criminal History check

New applicants need to complete the Queensland Health Criminal History Consent Form [PDF 315.48 KB]. Please print and sign this form in the presence of a witness who is at least 18 years of age and can verify your identity. Note, the form can be completed electronically but must have your handwritten (wet) signature. Certification is not requried.

Vaccination evidence

Current employees and new applicants need to provide evidence of vaccination or non-susceptibility for any, or all, of the specified vaccine preventable diseases. Certification is not requried.

This includes pathology testing results for the following vaccine preventable diseases:

  • hepatitis B
  • measles
  • mumps
  • rubella
  • varicella
  • pertussis.

There is no time limitation or expiration for serology results. It is important to note that in the case of pertussis, there is no acceptable evidence of immunity. You must provide evidence that you’ve had a pertussis-containing vaccine within the past 10 years.

Note that if your document needs translation, this must be done prior to the certification.

If you don’t have Australian citizenship or permanent residency, your employment will be for a specific amount of time, depending on your contract. We cannot guarantee employment after the end date of your contract, notwithstanding the fact that your working visa may be for a different period of time.

Get your documents certified

Before you upload your supporting documents, you’ll need to get them certified. A justice of the peace, commissioner of declarations, barrister, solicitor, or notary public can certify them.

If your documents need to be translated, translation should occur prior to the certification. This ensures that the certification uploaded to your application is the final version of the document.

If you get a position, your employing hospital may ask you to provide your original documents or certified copies for verification.

Hints and Tips

  • All files uploaded to your application must be saved in PDF format and ensure they are no larger than 2MB.
  • If you need to upload two or more documents as ONE file, scan all documents together, save as PDF format and upload to your application. (Note: these must not exceed 2MB)
  • Proof of Identity documents must be current and must include at least one type of photographic ID and identification that contains a signature and date of birth, this must also be certified.
  • If your ‘Surname’ does not match on your application or documents, a Marriage Certificate/Change of Name Certificate is required to be uploaded, this must also be certified.
  • If a document is not in English, an English translation from a translator approved by the National Accreditation Authority for Translators and Interpreters (NAATI) is required and once translated, it must be certified.
  • If you are unable to obtain certification, upload what you have and at a later date, upload via personal details or email a PDF copy to rmo-recruitment@health.qld.gov.au to be uploaded to your application on your behalf. Note that documents must meet  above criteria.
