Vaccine preventable diseases evidence

We’ll ask you to provide evidence you’ve been vaccinated against certain diseases before you begin working with us.

You can talk to your hiring manager or placement coordinator if you're:

  • not sure how to provide your evidence
  • can't meet the vaccination requirements.

If you're a student, talk to your education provider.

Documenting your evidence

Use the vaccine preventable disease evidence and certification form [PDF 549 KB] to document your evidence.

  1. Check your vaccine preventable disease role requirements.
  2. Read and sign the consent in section 1.
  3. Use your vaccine preventable disease (VPD) evidence and fill in section 2 yourself or ask your vaccine service provider (usually a GP) to provide your evidence and fill in section 3 for you.
  4. Return section 1 and section 2 or section 3 of your VPD evidence and certification form, including copies of your evidence.

Types of evidence

You need to provide a copy of one of the acceptable types of evidence for each VPD, which may include any of the following.

Proof of vaccination

  • A vaccine record book or card showing the vaccine and clinic details
  • An Australian Immunisation Register (AIR) history statement
  • A letter or statement from a nurse practitioner, immunisation nurse, vaccine service provider or another health professional confirming you were given the vaccine

Acceptable proof of immunity (not including whooping cough)

  • Pathology test results
  • A letter from a nurse practitioner or health professional confirming past infection with chicken pox or shingles
  • A letter confirming non-susceptibility to hepatitis B
  • You were born before 1966, for measles, mumps, and rubella only

If you don’t have records

  1. Visit your vaccine provider (usually a GP) to get vaccinated or tested.
  2. Ask them to fill in section 3 of the vaccine preventable disease evidence and certification form.
